Candela GentleMax Pro Plus: dual-wavelength versatility

The Candela GentleMax Pro Plus combines two distinct laser wavelengths on a single platform: a 755 nm alexandrite laser and a 1064 nm Nd:YAG laser. This dual-wavelength design is not common. Most devices offer one or the other, or they rely on a broad-spectrum light source such as IPL.

The 755 nm alexandrite wavelength is highly absorbed by melanin and is exceptionally effective for lighter skin types (Fitzpatrick I–III) and for finer to medium-coarse dark hair. It delivers energy efficiently to the follicle with a relatively shallow penetration depth, which makes it precise and fast for appropriate candidates.

The 1064 nm Nd:YAG wavelength penetrates more deeply into the skin with less absorption by epidermal melanin. This makes it the safer and more effective choice for medium-to-deep skin tones (Fitzpatrick IV–VI), for patients with recent sun exposure, and for treating deeper or more resistant follicles. It is also the wavelength of choice for darker skin because the risk of epidermal pigmentary change is lower.

Having both wavelengths on one device means I can select the optimal laser for each patient, and even for each body area, rather than forcing every patient into the same treatment box. That flexibility is central to how PBCM approaches individualized care.

Soprano Ice: a diode alternative

The Soprano Ice is a popular diode laser platform (typically 810 nm) marketed heavily in med spas. Its main selling point is a sweeping in-motion technique that treats large areas rapidly with lower energy delivered over a longer contact time. For some patients with ideal skin-and-hair contrast, this can produce acceptable results.

However, the Soprano Ice offers a single wavelength. It does not have the alexandrite option for lighter skin types or the Nd:YAG option for darker skin tones. The in-motion technique, while comfortable, sometimes requires more sessions to achieve the same follicular reduction because the energy per pulse is lower. For patients with hormonal hair growth, PCOS, or darker skin, the lack of wavelength flexibility is a meaningful limitation.

In my view, speed and comfort marketing should not override the ability to match the right wavelength to the patient's skin type and hair characteristics. That is why PBCM does not use the Soprano Ice.

IPL devices: not a true laser

Intense Pulsed Light (IPL) devices emit a broad spectrum of light rather than a single, focused wavelength. They are common in salons and lower-tier aesthetic settings because the devices are less expensive and treat large areas quickly. But IPL is not a laser, and the physics are fundamentally different.

Because IPL scatters light across many wavelengths, it is less selectively absorbed by the hair follicle. More energy is absorbed by the surrounding skin, which increases the risk of burns, pigment changes, and incomplete results, especially in darker skin types or in patients with recent sun exposure. IPL can work for a narrow subset of ideal candidates, but it is not a safe or effective primary technology for a medical practice serving a diverse patient population.

At PBCM, we do not use IPL for hair removal. The risk profile and lack of wavelength specificity do not meet the standard I set for physician-led treatment.

The Dynamic Cooling Device advantage

The Candela GentleMax Pro Plus includes an integrated Dynamic Cooling Device (DCD) that sprays a fine burst of cryogen onto the skin milliseconds before each laser pulse. This cryogen cooling protects the epidermis, reduces discomfort, and allows higher, more effective energy settings to be delivered safely to the follicle.

Cooling is not a luxury feature; it is a safety and efficacy mechanism. Without adequate epidermal protection, operators must use lower energy settings to avoid burns, which means less effective treatment per session and more sessions overall. Contact cooling (gels or chilled sapphire tips) offers some protection, but cryogen cooling is more precise and more effective at maintaining the temperature gradient needed for aggressive yet safe treatment.

This matters especially for facial treatments, where skin is thinner and more sensitive, and for patients with darker skin tones, where epidermal protection is critical to preventing post-inflammatory pigmentary change.

Physician-led parameter selection

Device technology is only as good as the clinical judgment operating it. At PBCM, every treatment plan is built during a consultation where I evaluate Fitzpatrick skin type, hair color and density, treatment area, medical history, medications, hormonal context, and recent sun exposure. The Candela platform then lets me translate that evaluation into specific wavelength, spot size, pulse duration, and energy settings.

A patient with PCOS and dark hair on fair skin may receive 755 nm alexandrite at aggressive settings with cryogen protection. A patient with richly melanized skin and coarse body hair may receive 1064 nm Nd:YAG at longer pulse durations with the same integrated cooling. The device supports the decision; it does not make it for me.

That is the difference between a med spa selling sessions and a physician-led practice building a plan. The laser is a tool. The medicine is in the reasoning.
