Melasma (the symmetric brown patches that appear most often on the cheeks, forehead, upper lip, and jaw) is one of the most misunderstood pigmentation problems in cosmetic medicine. It is sun-driven, hormone-sensitive, often hereditary, and quietly worsened by overly aggressive treatment. Heading into a Florida summer in 2026, we are already seeing the seasonal flare pattern in the Wellington practice, and the patients who do best are the ones who committed to daily protection and layered treatment in the winter. Why melasma is different from sun spots

Sun spots are discrete, lighter at the edges, and respond predictably to focused laser. Melasma is a chronic, symmetric, hormonally influenced pigmentation pattern that can flare with sun exposure, pregnancy, oral contraceptives, hormonal shifts, and heat. Treating melasma like sun spots (with aggressive lasers) frequently makes it worse, sometimes permanently.

What actually moves melasma

The foundation is daily, tinted, mineral sun protection: every day, not just sunny days. On top of that, a carefully chosen medical-grade brightening regimen (ZO Skin Health is the line we use most often) makes the biggest single difference for most patients. Gentle resurfacing peels chosen for the patient's skin type may be added once the regimen is established.

Laser plays a role only after the skin is calmed, brightened, and well sun-protected, and only with conservative settings appropriate for melasma. There is no laser that 'cures' melasma in one session, and we will tell you so honestly.

Why we avoid aggressive treatment

Heat- and inflammation-heavy treatments (strong IPL, aggressive resurfacing lasers, harsh peels) can trigger rebound darkening that is harder to resolve than the original melasma. Patients often come to us after this exact experience elsewhere. The reversal is slow and starts with backing off, repairing the barrier, and rebuilding the protocol.

The hormone and lifestyle factors we discuss

Oral contraceptives, hormone therapy, pregnancy, thyroid status, and even chronic heat exposure (running outdoors at midday in Florida, hot yoga, daily long commutes in a sunlit car) can drive melasma. We talk through what is realistic to change and what is not, and design a plan around it. As an endocrinologist, I can also evaluate hormone patterns when the history points there.

What a realistic timeline looks like

Most patients see meaningful improvement over three to six months of consistent home care plus periodic in-office support. Maintenance is forever: melasma is chronic, and protection plus skincare is what keeps it controlled. The goal is even, calm skin tone you do not have to think about every morning.
