The Problem

Why most PRP for hair disappoints

Patients arrive at our Wellington office having already tried PRP somewhere else more often than you might expect. They paid for a package, sat through the sessions, and finished the year with hair that looked roughly the same. The conclusion they usually draw is that PRP does not work. The more accurate conclusion is that PRP was delivered to them as a product rather than as medicine, and the three reasons it underperforms are consistent enough that they are worth naming plainly.

1. Weak single-spin preparation

PRP is not a standardized product. What ends up in the syringe depends entirely on the system used to prepare it, and inexpensive single-spin tubes produce a dilute concentrate that still carries a substantial load of red blood cells. Platelets are the point of the treatment, because they carry the growth factors that signal the follicle. A preparation with a modest platelet concentration is delivering a modest signal, and no injection technique compensates for that. Two clinics can both accurately say they offer PRP and be delivering materially different treatments.

2. No evaluation of why the hair is thinning

Hair loss is a symptom, not a diagnosis. Thyroid dysfunction, androgen patterns, iron and vitamin D status, insulin resistance, thyroid medication changes, postpartum shifts, rapid weight loss, and several common prescriptions all suppress hair growth, frequently more than one at a time. A treatment aimed at the scalp while an active internal driver goes unexamined is working against a headwind. That is not a failure of PRP. It is a failure to ask the medical question first.

3. No structured protocol and no maintenance plan

Hair follicles cycle over months, so a treatment plan built around whenever the patient happens to rebook does not match the biology. Nor does a plan that ends the moment the initial package is used up. Follicles that respond to PRP need continued signal, and the patients who hold their gains are the ones on a defined cadence with a maintenance schedule set in advance. Without that structure, the early response fades and it looks, from the outside, like the treatment simply stopped working.

The protocol below exists because each of those three failure modes is fixable. None of them requires a new technology. They require a physician designing the plan, a serious preparation system, and the discipline to follow a schedule.

Step 1

Medical evaluation, before anything is injected

Every protocol starts with a consultation that looks like an endocrine visit rather than a sales appointment. Dr. Anna Frisch, MD, PhD, is a board-certified endocrinologist with more than 26 years in practice, and the evaluation she designed reflects that training: history first, pattern of loss second, and laboratory assessment where it is clinically indicated.

We take a careful history of when the shedding began, whether it followed a pregnancy, an illness, a surgery, a period of rapid weight loss, or a medication change. We look at the pattern itself, because diffuse thinning, a widening part, temporal recession, and patchy loss point in different directions. Where the history warrants it, screening may include thyroid function, androgen markers, ferritin and iron studies, vitamin D, and metabolic markers such as fasting glucose and insulin.

We do not promise that a cause will be found. Hair loss is frequently multifactorial, and in a meaningful number of patients the evaluation returns nothing dramatic and the genetic pattern is the dominant story. That is still useful information, because it changes what we expect from treatment and how we talk about the timeline. What we will not do is skip the question. When a correctable contributor is present, addressing it makes every subsequent session more productive, and when it is absent, you proceed knowing rather than guessing.

This step is also where candidacy is decided honestly. Some patients are better served by a dermatology or surgical referral, and where that is the right recommendation, that is what you will hear at the visit. Step 2

Formulation on the EmCyte PurePRP SupraPhysiologic system

Once the evaluation is complete, the second variable we control is what is actually in the syringe. Your blood is drawn in-office and concentrated on the EmCyte PurePRP SupraPhysiologic system, an advanced FDA-cleared concentrating system used at the point of care. It is a deliberate choice and a more expensive one than the alternatives, and it is the reason our preparation is not comparable to a basic single-spin tube.

Supraphysiologic platelet concentration

The system concentrates platelets well above their baseline level in whole blood, which means more of the growth factors that signal the follicle are delivered in each session.

Over 99% red blood cell removal

Red blood cells are contaminant, not therapy. Removing more than 99% of them produces a cleaner preparation and, in most patients, a more comfortable treatment and a calmer recovery.

Customization per treatment area

The formulation is adjusted per patient and per area treated, including neutrophil-poor preparations where delicate work calls for them. Scalp, hairline, and facial applications are not interchangeable.

Closed-system sterility

Processing happens in a fully closed system at the point of care. Your blood is never exposed to open air between the draw and the treatment.

To be precise about the regulatory language, because it matters: the system used to prepare your PRP is FDA-cleared as a concentrating device. PRP is not FDA-approved as a treatment for hair loss, and any practice telling you otherwise is being careless with words. We would rather describe the treatment accurately and let the preparation quality speak for itself.

Step 3

A structured, physician-supervised treatment series

Treatment runs on a defined cadence rather than an open calendar. Most patients begin with a series of three to four sessions spaced roughly four to six weeks apart, timed to the follicle cycle rather than to convenience. Every session is delivered under physician supervision and follows the protocol Dr. Frisch designed, which means the depth, the pattern of placement, and the volume are consistent from visit to visit instead of improvised.

The intent of the series is to reawaken dormant follicles and encourage thicker growth from follicles that have miniaturized. Growth factors delivered into the scalp signal the follicle environment, and because follicles cycle over months, the response accumulates across sessions rather than appearing after one. A single treatment is a sample, not a plan.

There is no surgery and no meaningful downtime. A session takes under an hour from draw to finish, topical numbing is used, and most patients return to work or the rest of their day immediately afterward. Mild tenderness or pinkness of the scalp for a day is the usual extent of it.

Progress is documented rather than remembered. We photograph under consistent conditions at intervals, because the change is gradual enough that patients living with their own scalp every day are poor judges of it. Most people notice reduced shedding first, with density changes visible over three to six months. Individual results vary, and where a patient is not responding as expected, that becomes the conversation rather than another quiet round of the same treatment.

Step 4

Maintenance, because hair keeps cycling

The initial series is the beginning of the plan, not the whole of it. Follicles continue to cycle, and the underlying tendency toward thinning does not disappear because a series has ended. Maintenance sessions, typically scheduled at intervals set for your response, keep the signal going and are the main reason some patients hold their gains for years while others watch them fade within months.

Where appropriate, maintenance also includes physician-directed support beyond the scalp itself. That may mean nutraceutical support, correcting an iron or vitamin D deficiency the evaluation identified, addressing thyroid management alongside your treating physician, or medical therapies chosen for your situation. These are recommendations made case by case at a medical visit, not a fixed retail bundle.

Because the same regenerative logic applies to skin, some patients continue maintenance across both. Our PRP and PRF services cover facial and under-eye applications, our Wellington PRP page covers the local logistics and pricing structure, and the Collagen Rebuild Protocol is the skin counterpart to this program.

Who Designed It

Designed and supervised by Dr. Anna Frisch, MD, PhD

Dr. Anna Frisch is a board-certified endocrinologist with an MD and a PhD, more than 26 years of clinical practice, and over 40 scientific and medical publications. She built the Root Cause Hair Restoration Protocol the way she builds any endocrine treatment plan: identify what is driving the problem, choose an intervention that matches it, deliver it on a schedule, and measure the response.

She designs and supervises every protocol at Palm Beach Cosmetic Medicine, and the clinical standards, the preparation system, and the treatment cadence are hers. That is the difference this page is really describing. Hair loss is a medical question, and it deserves a medical answer. Read more about Dr. Frisch.

Root Cause Protocol

What patients ask about PRP hair restoration

How many PRP sessions will I need?
Most patients begin with a structured series of three to four sessions spaced about four to six weeks apart, then move into maintenance. The exact number is set by your physician at the consultation based on the pattern of thinning, how long it has been present, and what the evaluation finds. Individual results vary.
When do results appear?
Hair grows slowly, so change is gradual. Most patients notice reduced shedding first, often within the first couple of months, with visible density changes appearing over three to six months as the follicle cycle turns over. Individual results vary, and we photograph at intervals rather than relying on memory.
Does PRP for hair loss hurt?
Scalp injections are uncomfortable rather than painful for most patients. We use topical numbing and a technique designed for comfort, and the session itself is short. There is no surgery and no meaningful downtime, so most people return to their normal day right afterward.
Who is a candidate for the Root Cause Hair Restoration Protocol?
Adults with thinning, widening part lines, reduced density, or increased shedding are the usual candidates, including women in the postpartum and perimenopausal windows and men with early to moderate pattern thinning. Candidacy is confirmed by a physician. Where PRP is not the right tool, or where surgical referral is the honest recommendation, that is what you will hear.
Why does the endocrine evaluation matter for hair loss?
Thyroid function, androgens, iron and vitamin D status, insulin resistance, medications, and recent weight loss all influence hair growth. Treating the scalp while one of those drivers is active produces a partial result. We screen for them where clinically indicated first, so the treatment is working with your physiology rather than against it. We cannot promise a cause will always be identified.
How is your PRP different from a med spa PRP treatment?
Two differences. First, the evaluation: your protocol is designed by a board-certified endocrinologist and begins with the medical question of why the hair is thinning. Second, the preparation: our PRP is concentrated on the EmCyte PurePRP SupraPhysiologic system, an advanced FDA-cleared concentrating system, with over 99% of red blood cells removed and the formulation adjusted per treatment area.
Is PRP FDA-approved for hair loss?
No. PRP is prepared using an advanced FDA-cleared concentrating system, and it is used for hair restoration as a physician-directed treatment. We describe it accurately rather than overstating it, and we tell patients plainly that individual results vary.
What does the protocol cost?
Pricing depends on the number of sessions planned and whether other treatments belong in the plan, so it is quoted at your consultation rather than sold as a pre-paid package. Book a consultation or call (561) 600-8707 and we will walk you through the plan and the cost together.
